I have the same problem on two different PCs (XP and Win 7) using Outlook 2003 & 2007. Both have the ‘Show this folder etc’ greyed-out despite the address book service being in place and therefore can’t be Added to although various drop-downs are empty within the Address Book itself

This did not work for me. I used easy transfer cable by Belkin – moved .pst from Vista to Win 7 – both using Outlook 2007 with BCM. Though Contacts folder transfered for both Outlook and BCM, neither address book was populated. Went thru steps you suggested – did not seem to refer to 2007 version. Neither Microsoft nor Belkin provide a workable solution. Belkin denies that problem is realted to their product – but I have seen others with similar complaint – so far none of recommended fixes has worked for me.
